Registering as a new patient?
To register on our list, you must live in our catchment area and can prove that you have lived in this country for over six months. You will be asked for proof of residence. Under no circumstances will this practice refuse an application based on your gender, social class, age, religion, disability, appearance or medical condition. Our policy on this matter is outlined below.
To register on the surgery list, you must fill in all the forms below and provide your NHS number and name of your previous surgery on them. You will also be asked to provide proof of your identity and proof of your residence. Please bring your registration forms into the surgery between 1030 and 1200 noon ONLY M-F, when our staffing levels will be able to provide you with necessary time to check you have completed all the information required. Please note that due to the notice of a local surgery 3 months ago, patients that are now requiring registration, may have to wait a little longer for this process to be completed. We would remind you that this is a zero tolerance practice, and would ask that you bear with us - we are working as quickly as we can.
If you need to see a doctor before the registration process is complete, please enquire at the reception.
Please ensure you obtain enough medication from your current surgery to last until you are registered with this practice.

Named Accountable GP
Each patient registered with this Practice has been allocated a named GP who is responsible for their overall care at this surgery. Patients can be seen by either of the Partners at the practice when booking an appointment. Patients can express a preference to be seen by a particular GP.
